Instructor Required Paperwork

Documents we need for your file

In this section you will have to submit the following documents to complete your file:

• IRS Form W9
• A clear photo of yourself with an animal as a jpg or png
• 3-4 sentence biography describing your professional animal experience and what makes you an expert in animal emergencies. (Please write the bio in third person as if you were speaking about another person).
• Proof that you have purchased the required instructor supplies. There is a link in this lesson to purchase your kit and you must upload the receipt to verify with the training manager.
